Referrals to the Medical Home Clinic

To help you know if your child may benefit from the Medical Home Clinic, consider the following questions:

• Are you coming to Arkansas Children's Hospital more than three times a month?
• Would having two appointments on the same day help meet your needs?
• Would it be helpful to have a written individual care plan for your child to take to other doctor appointments?

If you answered yes to any of these questions, let the Medical Home Program team schedule an appointment for your child. Your local doctor can refer you to us by calling 501-364-3030.
