Yes, there is a place on the "Shadowing Confidentiality and Attestation Agreement" form for your parent or guardian to sign.
We ask that you turn in your registration form no earlier than 3 months, but not later than 3 weeks prior to your requested date. You will be contacted approximately 2 weeks prior to your requested shadow date to inform you if we are able to meet your request to shadow.
There are certain hospital departments that will not accept shadowing participants. We will make every effort to accommodate you, but cannot guarantee that we will be able to match you with a certain area/department.

You should call the Volunteer Services office at 501-364-1825. We will try to work with you to schedule another time for your shadowing experience.
Students and adults will receive an email confirmation that will include details of where to meet and also confirmation of the dates we were able to schedule your shadowing experience.
One TB skin test result is required in order to shadow. You will have a TB skin test administered and have that read in 48 to 72 hours (you will be told when to return to have your test read). TB skin tests are good for one year.
You can go to your physician, a clinic or Arkansas Children's Hospital's Occupational Health. Students under the age of 18 must have a parent or guardian with them.
You may drop in to Arkansas Children's Hospital Occupational Health without an appointment on Tuesdays from 6:00 am to 4:00 pm.
Occupational Health is located in our Human Resources building at 1919 West 12th Street. ACH is glad to provide this service to you at no charge through our Occupational Health Department. If their times are not convenient for you, you may want to go to your physician or your community clinic.
