The Medical Home Clinic for Special Needs Children is a clinic for children with several chronic medical conditions that need the services of many sub-specialty clinics at Arkansas Children’s Hospital. The goals of the Medical Home Clinic for Special Needs Children include:
Assist in your child’s general medical care
Assist in meeting the nutritional needs of your child
Assist you in obtaining appropriate developmental services for your child
Help you with a better understanding of your child’s care needs
Assist in the coordination of care with your local doctor, the Arkansas Children’s Hospital doctors and other providers for your child
Decrease the number of visits to the emergency room
Provide support and guidance for parents coping with their child’s chronic illness through emotional support and behavioral management
Referrals to the Medical Home Clinic
To help you know if your child may benefit from the Medical Home Clinic, consider the following questions:
Are you coming to Arkansas Children’s Hospital more than three times a month?
Would having two appointments on the same day help meet your needs?
Would it be helpful to have a written individual care plan for your child to take to other doctor appointments?
If you answered yes to any of these questions, let the Medical Home Program team schedule an appointment for your child. Your local doctor can refer you to us by calling 501-364-3030.
