The Arkansas Children’s Care Network (ACCN) Family Advisory Council (FAC), established in 2021, serves as an advisory resource committee to the Care Management Team of ACCN to provide constructive input to assist in the advancement of the ACCN’s guiding principle of improving the health of children across Arkansas by enabling high quality, cost-effective & coordinated care close to home.

Care Management

Care management is a nursing service that provides medical and social support to patients and families with chronic or complex health and social needs to improve health outcomes. ACCN care managers engage and partner with pediatricians and their pediatric patients and families across the state of Arkansas to provide excellent, consistent, and timely patient-centered and coordinated care.

CMFAC Goals:

  • To offer ideas and suggestions regarding care management services that affect patient and family-centered care.
  • To review recommendations and concerns referred to by parents, staff, physicians, or Administration.
  • To serve in an educational and advisory role, as needed or requested, regarding patient/family perceptions of care and services.
  • To help us partner with your pediatricians to provide ideal health care experiences and transitions of care.
  • Offer a wide variety of diverse perspectives.

CMFAC Advisors

The FAC consists of parents or guardians of ACCN patients interested in providing meaningful feedback on Care Management experiences and promoting a broad agenda focused on enhancing Care Management services. (The focus of conversations will be around care management specific activities. Any clinic-level concerns or suggestions will be related to the respective clinics for review.)

Time Commitment

The CMFAC will meet on a virtual platform quarterly.

Responsibilities

The CMFAC consists of parents or guardians of ACCN patients interested in providing meaningful feedback on Care Management experiences and promoting a broad agenda focused on enhancing Care Management services.

Responsibilities
  • Elected parent advisor who coordinates & directs the work of the Council.
  • Works with the facilitator to develop a plan for the Council.
  • Participates with other members in contributing ideas, interpreting information & making decisions.
  • The Chair Person guides & supports the Council through the meeting process to achieve the objectives.
